Why John Piper refer to himself as Dr. John Piper? It is a doctorate other than a PhD?

I realized I never referred to John Piper himself as Dr. John Piper, even though he has a Ph.D. from the University of Munich. Is it something spiritual, or have a doctorate degree does not entitle to be a doctor so and so? Besides, I was reading the biographies of the teachers college, I'm going to (Baptist Bible College in Clarks Summit, PA) in January and I realized most of the professors in the Department of doctorates, but only heads and teachers in the education department have PhDs. What is the difference between a doctorate and a PhD?? and in an unrelated note, it is dishonest to a chiropractor to call themselves Dr. so and so in public life? It seems dishonest to me.

You do not have to call your doctor if you do not want. Plenty of doctors do not use their title. A Ph.D. is a doctorate in philosophy. Simply fulfilling the task of a physician, as someone in a hospital can do (or what a chiropractor seeks to do) does not allow you to call the doctor yourself.
